For your convenience, our dedicated referral fax line is 626-722-5095. Please note that each admission to Aurora Charter Oak is handled on a case-by-case basis. Please contact our 24-hour Assessment Center should you have any questions regarding possible exclusions for admission.
If you would like to discuss cross-referrals with our facility, we invite you to contact our Business Development Department at 626-214-2029. Our Business Development staff is available to visit you at your practice location, or to arrange for a visit to our facility.

Family/Friend Referral
Family members and friends refer a large percentage of individuals to Aurora Charter Oak. When you know someone close to you who is struggling with a mental health or substance abuse problem, you are in the most important position to provide encouragement and support. Family members or friends are welcome to call Aurora Charter Oak to ask about getting help for their loved one.
Perhaps you are concerned about a family member who is using drugs or drinking heavily, or who appears to be depressed? Our specially trained Assessment Center staff are available 24 hours a day, 7 days a week to answer your questions and help you decide on the next step.

Self-Referral
